Registration

  • This registration is limited to the first 2 State Agency registrants from the same State Agency ($600.00 per each registrant). Register the first SA attendee in the main form. Register the 2nd attendee as a guest.
  • Additional SA registrants ($500.00/person), after the first 2, from the same SA. Please register the 3rd SA attendee in the main form. Register additional SA attendees as guest.
  • Free registrations for first 10 CMS attendees. Register each attendee as a guest on this form after first attendee is added.
  • Registration for CMS attendees after first 10 registrations.
  • AHCA Member registration. You must be an active member of AHCA to receive this registration fee. This fee is subject to change if less than 5 AHCA members register.
  • This fee is reserved for CMS Contractors. This price is per person. If you wish to register 5 or more from the same company please email contact@ahfsa.org

2. Check: To pay by check please download the AHFSA 2018 annual conference registration form HERE. This form can be returned by email to me at tfoy@firstpointresources.com or returned by mail along with your check payment to:

Attendance Cancellation Policy: 

 For Non-Members:  Full refund up will be given, less a $85.00 administration fee, if cancellation is received in writing to AHFSA by September 1, 2018. Cancellation on 8/1/17 or later will result in a refund of 1/2 the registration fee minus a $85.00 administration fee.

AHFSA will allow an organization to swap out attendees at no additional cost.

For Members: A full refund, less a $85.00 administration fee, will be given if cancelled by 8/16/2018. In the case of a personal or family emergency a full refund will be given at any time, minus a $85.00 administration fee.
