The Association of Health Facility Survey Agencies invites you to our 41st annual conference. Our Program will enhance your professional experience and exploration well beyond what each could do singularly. Join us in Seattle, Washington and enjoy your colleagues and the wonderful city of Seattle.

Conference Objectives
: To bring practical and relevant information to State survey agency managers and directors on Federal requirements, legal issues, emerging issues in healthcare delivery within health facilities, and survey concerns.   To network with other agencies for improving the quality of care and services provided by health care facilities, agencies, and programs by ensuring the established standards and regulations are met.

Conference Location: The conference will be held at the Grand Hyatt, 721 Pine Street, Seattle, Washington 98101.  A block of sleeping rooms has been arranged for this conference.  When making hotel reservations mention that you are attending the Association of Health Facility Survey Agencies (AHFSA) 41st Annual Training Conference in order to receive the special lodging rate.  There is a single or double occupancy rate not to exceed $129 plus tax. 
 

The cut off date for reservations is Friday, September 16, 2011. The  Hotel web site is for reservations https://resweb.passkey.com/go/health2011 .  A one-night deposit is required for all reservations.  Reservation cancellations must be made 24 hours prior to the convention date to avoid a no-show fee of one night’s room and tax being charged.    For extended stay reservations a $50.00 early departure fee will be charged unless the hotel is notified at or before check-in of any change in the planned length of stay.

Conference Registration Policy: All conference registration fees will be refunded with a written cancellation request postmarked by September 13, 2010. No refunds will be issued for cancellations made after September 16, 2011. The fees remain the same if you are unable to attend and wish to make a substitution.

Ground Transportation: There is shuttle service from the Seattle Airport to the Grand Hyatt Downtown. 

Special Assistance: We will be happy to assist you with any special needs (I.e., physical, dietary).

Additional Information: Recommended dress is business casual.
